An applicant for a license to practice as a licensed practical nurse shall submit to the board written evidence, that the applicant:

D. Examination Information:

Cost: $200.00 The National Council Licensure Examination (NCLEX) is given 6 days a week. An applicant for a license to practice as a licensed practical nurse shall pay a fee prescribed by the board to the department at the time the application is submitted. An applicant for a license to practice as a practical nurse is required to pass the National Council of State Boards of Nursing (NCSBN) - National Council Licensure Examination (NCLEX) exam or another Board approved licensing examination. On successfully passing the examination, the board shall issue to the applicant a license to practice as a licensed practical nurse. |

All licenses expire 12/31 of even numbered years, and must be renewed by that time.

F. Renewal Fee:
$100.00 per renewal
period $200.00 if received after 12/31 of renewal period |
